Proof of Federal Tax Payer ID number or Social Security Number
- County Assumed Name Certificate Registration (filed at county of business recorder’s office) or State Certificate of Assumed Name Registration (filed at State Recorder’s office).
Partnership
Proof of Federal Tax Payer ID number or Social Security Number
Copy of the Partnership Agreement
Limited Liability Company (LLC)
Proof of Federal Tax Payer ID number
A copy of the Certificate of Organization/Existence (seal) or Filed Articles of Organization and LLC Agreement.
Corporation
Proof of Federal Tax Payer ID number
A copy of the Certificate of Organization/Existence (seal) or Filed Articles of Incorporation/Organization
Organization (Unincorporated Association)
